During the course of an investigation, it was determined that a child fell, and the caregivers did not know how the child had fallen and hurt their selves. On a separate occasion during the investigation inspection, a child had separated from the group and went into the science lab and was briefly unsupervised. When the child was returned back to the group, the caregivers were unaware the child had left the group.
